The procedure

The operation is performed under general anaesthesia and lasts between 1 hour and 1.5 hours.

Following the operation, you will need to have someone take you home and help and monitor you for the first 24 hours.

Where is the surgery performed?

Dr. Boudana only operates on patients in private state-of-the-art operating rooms accredited by Accreditation Canada. This accreditation is a guarantee that the facilities meet stringent safety and quality standards.

Dr. Boudana’s patients have the choice of having their surgery done in Laval or in Saguenay.

Consultation preparation

To properly prepare for the consultation, compile all your relevant medical and health information (list of medications, doctors’ reports, history of previous surgeries, mammogram reports, breast biopsies, etc.). Dr. Boudana will also ask about your medical history and your family history of breast cancer.

What happens in the consultation?

During the consultation, Dr. Boudana will listen carefully to your expectations regarding desired breast size and shape. We advise bringing in photographs from magazines or social media sites such as Realself, Facebook, and Instagram of the breast sizes and shapes you like (but beware that some of these photographs may have been digitally altered).

This will help Dr. Boudana better understand your expectations and goals. As such, a large part of the consultation is dedicated to listening to you.

However, not all goals are realistic, and it is important to understand what surgery can and cannot accomplish.

Dr. Boudana will examine your breasts and assess all the factors that can influence the aesthetic result. These include the size and shape of your natural breast, skin and gland quality, and the position and size of your nipples and areolas.

Dr. Boudana will then photograph your breasts from different angles and place these photographs in your medical record. These photographs will be kept strictly confidential.

He will then take a 3D scan of your breasts and create simulations to choose which implant size is most appropriate to achieve your goal. A virtual reality simulation is also offered.

Breast implant types

There are two types of breast implants: round implants and teardrop implants (also known as anatomical implants).

It has long been accepted that anatomical implants have a more natural look. However, recent studies suggest that both plastic surgeons and patients cannot tell the difference between breasts augmented with round or teardrop implants. As such, Dr. Boudana believes that, in the absence of sagging or breast deformity, round and teardrop implants will achieve the same result.

Cohesive silicone gel implants
These implants are made of a silicone elastomer outer shell filled with a cohesive silicone gel, meaning they retain their shape and roundness like a gel and are less likely to leak if the shell breaks.

These implants differ from saline-filled implants, which in thinner patients can ripple showing folds and wrinkles. The use of silicone implants may reduce the risk of rippling in some patients.

During the pre-operative consultation, Dr. Boudana will advise you on breast implant types, and discuss the pros and cons of each type with you. As an expert breast augmentation surgeon, he is comfortable using all types of implants: saline, silicone, and hybrid.

Saline implants are made of the same silicone elastomer shell as silicone implants but are filled with a saline solution. It is recognized that they are more likely to produce waves under the skin that may be visible especially in thin patients.

What is the lifespan of breast implants?

Dr. Boudana understands that breast augmentation represents a considerable investment. Therefore, he wants patients to be reassured about the quality and lifespan of breast implants. There is a misconception that breast implants should be replaced every 10 years, when in fact women can theoretically live their entire lives with the same implants. In reality, breast implants should only be changed if there is a problem.

In addition, implant manufacturers are so confident in the longevity of their product that they offer a lifetime guarantee on implants. This means that in the rare case an implant ruptures, the manufacturer will provide a replacement implant at no charge.

You should also note that in the rare case of rupture, cohesive gel and saline implants are particularly safe. While implants used decades ago had higher risks of rupture, today’s implants are safer. If a rupture does occur, saline implants contain a saline solution that will be absorbed by the body, while the solid silicone in a cohesive gel implant will not leak.

Furthermore, several studies have shown that women with breast implants are no more likely to develop breast cancer than women without implants.

Breast implants: over or under the muscle?

The breast implant can be placed under the muscle (subpectoral augmentation) or over the muscle (subglandular augmentation).

Many factors will influence this decision. These include the quality and thickness of the breast tissue, the strength of the pectoral muscle, the size and type of implants chosen, and the type of physical activity the patient performs.

All this will be discussed in consultation with Dr. Boudana, who will explain the advantages and disadvantages of subglandular and subpectoral augmentation.

Scars

There are three main approaches to breast augmentation:

Submammary route: the incision is located in the crease under the breast. This is often the most popular option for both patients and surgeons. It minimizes the risk of infection and capsular contracture while leaving a discreet and hidden scar in the mammary crease. The length of the scar is generally between 2 cm and 4 cm.

Periareolar route: the implant is introduced through an incision located on the lower part of the areola where the breast skin and the skin of the areola meet. This approach can be interesting for some patients who require a slight lifting of the areola and allows the insertion of the implant through a periareolar incision. The risks of capsular contracture are slightly higher with this approach and should be discussed in consultation.

Axillary approach: this approach allows the implant to be introduced through a very discreet incision in the armpit. The risk of contracture is also slightly higher than with the submammary approach, but this approach has many advantages for some patients.

Breast augmentation incision

Short scar breast augmentation

Short scar breast augmentation is one of Dr. Boudana’s specialties. In this procedure, saline implants are inserted in a submuscular position under the breast (incision under the breast). The scar in this case is generally between 1 cm and 2 cm long. If this technique interests you, you can ask for more information during your consultation.

What does the surgery entail?

The surgery is performed under general anaesthesia and lasts between 1 hour and 1.5 hours.

Following the operation, you will need someone to take you home and help and monitor you for the first 24 hours.

Once you are under anaesthesia, the skin of your breasts will be carefully cleaned with an antiseptic to prevent infection. This step is performed according to medical recommendations and standards. The procedure then begins with the incision. For better visualization, the surgeon uses fibre optic instruments to create a lodge for the prosthesis.

Implant placement is done using the latest techniques, which have been proven to be effective in minimizing the risk of complications:

  • Protection of the nipples by an impermeable film preventing the spread of bacteria from the mammary glands;
  • Irrigation of the implant with an antibiotic or antiseptic solution before opening the implant package;
  • Washing of the prosthetic lodge with the antibiotic or antiseptic solution;
  • No touch technique: the implant is slipped into a specific pocket for insertion;
  • Multi-plane closure.

The use of an insertion pocket (Keller Funnel type) allows for a smaller incision, even for larger implants.

After surgery

After the surgery, you will receive a personalized follow-up. Our nurses will also be available and will perform the first dressing change at our clinic, typically one week after surgery.

You will be given a postoperative instruction sheet when you are discharged as well as a prescription for pain medication.
